Outreach Supporting Individuals Advance Care Planning

This session will enable the attendee to understand the principles and practices for planning end of life care.  Attendees will learn strategies which will encourage and empower open discussion enabling the person to identify and document their wishes and preferences. We will explore the key documents including Advance Care Plans, Advance Decision to Refuse Treatment and the Treatment Escalation Plan. Skills learned in this session support the person to develop their own end of life care plan and are therefore relevant to all settings. Aims: To develop confidence to support individuals and families when discussing their wishes and preferences. To understand the relevance of Advance Care Planning to best interests decisions To understand the legal difference between Advance Care Plan and Advanced Decision to Refuse Treatment To be familiar with local Advance Care Plan documentation and associated documents “Brilliant, thank you.

” Entry requirements: There are no prerequisites for this course. This course is designed for the non-registered workforce.Time: Registration from 12.45pm. Course runs 1 – 4.30pmDates: Wednesday 30th January 2019 and Wednesday 19th June 2019 at North Devon Hospice, Barnstaple This session is also being held at Hatchmoor Nursing Home in Torrington on Wednesday 30th January 2019. Failure to give 5 working days notice or non-attendance will incur a £25 fee per person to cover administration costs, unfortunately as we are a charity this fee is non-negotiable.
